Under Evaluation Setup - open Setup 

Form structure (see Figure 1)

The following fields must be filled in:

Period for the previous year

Period for the current year

Total points => the higher this value, the more detailed the result

Criterion Grades up to (recommended: 6)

Create evaluation items based on business criteria → yes/no

Appearance

Delivery time

Delivered quantity

PPM (Parts Per Million)

The corresponding numbers for the business criteria must be assigned.
